The extended care team work closely with General Practice to support their patients in the community.

Our commitment to partnership with local iwi, social and health providers ensures that our team are able to access appropriate services for each individual and their whānau.

General practice and the extended care team work together as one to support the health of our patients in the community.

We require a referral from your general practice or other relevant health providers. The criteria for our service ensure we support patients who have the greatest need.

We are a FREE service but we do require a referral from your GP or practice nurse. Please phone your GP surgery to discuss our service as an option.

Advocacy

Kaiārahi - Peer Support Worker Our peer support worker links people living with a chronic condition such as diabetes and/or other long-term conditions with peers who are able to share similar knowledge and experiences. Kaiāwhina - Community Health Worker Our Kaiawhina works with high-needs patients with a specific focus on Maori, Pacific Island and others at risk. The kaiawhina acts as a link between patients and their whanau to the practice, the community and other service providers. Waiora Manaaki/Health Coach Our Waiora Manaaki works collaboratively as a member of the general practice team in the Taupō/Turangi region. They encourage patients to identify their own priorities for change, acting as a bridge between patients and clinicians. As well as supporting patients to self-manage and providing emotional support, they navigate the health and social services system for patients and their whānau making connections to appropriate community supports and providing continuity within a busy primary care team.

As well as supporting patients to self-manage and providing emotional support, they navigate the health and social services system for patients and their whānau making connections to appropriate community supports and providing continuity within a busy primary care team.

Wellness / healthy lifestyle

Dietitian Our dietitian works together with patients living with long-term conditions to come up with realistic and achievable lifestyle changes to improve their nutritional health. Exercise Consultant Our exercise consultants support GPs and practice nurses recommending, designing and delivering a variety of exercise options for long-term condition patients. Health Improvement Practitioner (HIP) Our Health Improvement Practitioners, also known as HIPs, work as members of the general practice team in the Taupō/Turangi region to deliver focused brief interventions for anyone of any age whose thoughts, feelings or behaviours are impacting on their health and wellbeing.

Community Child Health Nurse Our aim is to provide maternal and child health nursing to high needs patients in the community to ensure all children are engaged with key health and social services and receiving their milestone checks.
