Description

Our team of physiotherapists, occupational therapists, a dietitian, a psychologist and a registered nurse provide a range of services including vocational rehabilitation, social, home and community-based rehabilitation, training for independence, concussion services, pain management, physiotherapy, dietetics, psychology and occupational health nursing services.  Occupational nursing services include pre-employment medicals, drug-testing, hazard based health monitoring and vaccinations.

Integrated Care Pathway – Musculoskeletal (ICP - MSK)

The Integrated Care Pathway is ACC’s innovative way of managing people with injuries that require coordinated rehabilitation services. Habit Health provides the platform for the care team (physios, specialists, return-to-work therapists and health navigators) to see patients under this contract and coordinate all the moving parts.
