Description

Auckland PHO provides free education and support programmes for people with type-2 diabetes and prediabetes through Diabetes Self-Management Education (DSME) workshops.

The workshops are designed to support people with type-2 diabetes and their whānau/support person(s).  We also welcome individuals who are prediabetic wanting to learn more about type-2 diabetes. This programme is part of a wider self-management initiative guided and funded by Te Toka Tumai Auckland (previously Auckland District Health Board). Our workshops are free of charge and delivered across Auckland communities on behalf of Procare, National Health Coalition and Alliance Health PHOs.

Our motivating one-day workshop aims to build people's knowledge, confidence, and skills around managing diabetes. It covers a broad range of topics.

While there are objectives and topics to cover, information is tailored as much as possible to each group. The workshops are led by our trained facilitators. They are informative, conversational, and patient-centred. We highly encourage whānau/support people to attend as self-management involves the whole family!

Staff

Our trained DSME facilitators are from a range of health professional backgrounds including dietitians, registered nurses, and health coaches. Our facilitators are from a range of cultural/ethnic backgrounds.

Anyone can access

Any individual with type-2 diabetes and/or their whānau are welcome to come along. We also welcome individuals who are prediabetic wanting to learn more about type-2 diabetes. Patients do not need to be enrolled with your practice/service to access this programme.

We run both community and online Zoom workshops. We can also run workshops by request to suit your community's needs.

Why refer to our Diabetes Self-Management support programme? Living with diabetes can be scary and confusing and feedback from our attendees is extremely positive; patients feel they are less afraid and more empowered having gained the knowledge and skills to make some major lifestyle changes with a better understanding of diabetes.
