Description

Green Prescription (GRx):

Green Prescription (GRx) is a free referral-based health and wellbeing service supporting adults, children, young people and whānau to become more physically active and improve their overall lifestyle.

The service is suitable for individuals who would like encouragement and motivation to increase physical activity, improve eating habits, manage a long-term health condition, support maternal wellbeing (including pre and post-natal), or improve their mental wellbeing if experiencing stress, low mood, anxiety or isolation.

We deliver support through face-to-face, phone and virtual consultations, as well as culturally responsive and community-based programmes.

Our services include:

  • One-on-one support from a Healthy Lifestyle Advisor (community, Telehealth or online)

  • Regular check-ins for up to 3 months

  • Active Families – family-focused support for children and young people

  • Maternal health and wellbeing support (including pre- and post-natal guidance)

  • Refugee health and wellbeing programme

  • South Asian community programme

  • Live walking groups across local communities

  • Aqua exercise classes

  • Live-streamed exercise classes

  • Nutrition and wellbeing webinars and workshops

  • SMS motivational support

  • Free translation and interpretation services

Our approach is supportive, non-judgemental and tailored to individual goals, culture and needs.

How do I access this service?

Referral

Green Prescription is a first-line lifestyle intervention supporting prevention, early risk management, maternal wellbeing, and long-term condition care across the lifespan.


Referral Criteria

Green Prescription (GRx) welcomes referrals for:

  • Adults (18+), children and young people (via Active Families)
  • Individuals or whānau who would benefit from becoming more physically active or improving nutrition — including preventive support before a diagnosis is made
  • Patients with emerging or early risk factors (e.g. rising HbA1c, increasing cardiovascular risk, weight gain, reduced activity levels)
  • People living with long-term health conditions who would benefit from structured lifestyle support alongside clinical care
  • Individuals experiencing stress, anxiety, low mood or social isolation
  • Pregnant and post-natal people requiring maternal wellbeing, safe physical activity and lifestyle guidance
  • Refugee and former refugee communities
  • South Asian individuals and families who would benefit from culturally tailored support

GRx is particularly effective when introduced early, before risk factors progress or lifestyle changes become harder to initiate. It can be used alongside clinical treatment plans or as proactive support to prevent future health complications.


How to Refer

  • Primary care referrers could send eReferrals via HealthLink forms (quick, secure and preferred pathway).
  • CareConnect is the preferred eReferral method for primary care. All systems, Medtech, MyPractice, Indici supported.
  • Regional clinical portal integration for secondary care.
  • All health professionals can refer.
  • Individuals and families can also self-refer by calling 0800 228 483 or completing the online self-referral form.

Active Families programme

Active Families is a free, whānau-focused programme supporting children and young people to develop lifelong healthy habits through fun, practical and community-based physical activity and wellbeing support. The programme is well suited to children that are inactive, above a healthy weight and would like regular motivation to make changes with their whanau. Active Families is for school aged children aged 5 - 17 years, along with the support from their parents and wider whanau. The programme helps families build confidence in being active together by providing personalised guidance from a Healthy Lifestyle Advisor, access to community exercise opportunities, and encouragement to move more in ways that suit each child’s age, interests and abilities. Active Families also supports nutrition awareness, wellbeing skills and culturally responsive lifestyle advice. Referrals are welcome from health professionals, schools, or families who wish to self-refer. PRIMARY CARE eReferrals

eReferrals can be made by Primary Care Health Professionals through HealthLink forms, using Care Connect or Care Select. 

Step 1 - Go to ‘Green Prescription’ in the CareConnect eReferral platform
Step 2 - Please provide a clear reason for the referral and give the patient a Green Prescription flyer
You can advise the patient they will be contacted within 4 working days

DHB / Secondary Care eReferrals

DHB based health professionals can easily send eReferrals through the Regional Clinical Portal / Concerto by using the 'Green Prescription' form.

If an eReferral cannot be made, a referral can be made through the Sport Auckland website HERE
