A service aimed at supporting individuals and families / tamariki, rangatahi, kaumatua, whānau with illness or disability. Individual health plans are developed and implemented, with the aim of improving health, independence and quality of life. We provide: Mental health education and support Health and fitness activities and education Wae wae clinics Mirimiri Smoking cessation Crisis intervention Advocacy and referrals Transport and mobility services Cultural support How to Access: Referral from health and social service providers/whānau/self-referral.
