Description

Green Prescription (GRx) is a FREE programme to improve the health and wellbeing of adults and whānau through physical activity/nutrition support to help individuals make sustainable, healthy behaviour changes. Through education, removal of barriers, and development of personal skills GRx aims to enhance physical, mental, and social wellbeing.

How we support

We can provide support to individuals or to families, adapting based on need (which might be as little as a one-off contact), but could also include:

  • Face-to-face consultations (in-homes, in the community or online) with a Programme Advisor, with regular follow-ups for up to 12-months
  • Resources and advice around healthy eating, physical activity, and general wellbeing
  • Motivation and goal-setting support
  • Practical cooking and food workshops
  • Physical activity tasters, and support to link into community events and physical activity programmes

How do I access this service?

Referral

You are welcome to self refer via our website, or you can ask your GP, practice nurse or other healthcare professionals to make a referral to Nuku Ora on your behalf. Once referred, our Programme Coordinator will contact you to arrange a consultation within five working days. 

Nuku Ora accepts referrals across Wellington, Porirua, Kapiti, Hutt Valley, and Wairarapa.

For self-referrals: Use our secure online Self-referral form.

For health care professionals: Use our secure online Healthcare Referral form, or send an eReferral directly from HealthLink or BPAC. We no longer accept faxed referrals.
