Our integrated care model combines expertise from across various specialties including home support, community nursing, primary healthcare, and workplace health. This wrap-around approach enables us to deliver better outcomes to enhance the wellbeing and independence of the people we serve.

 

By working in close partnership with our clients and their families, as well as other health professionals involved in their care, we can tailor our services to meet the unique needs of each individual—be it round-the-clock care or just a few hours a week.

Staff

Our staff members are the backbone of our organisation. We recognize their crucial role, which is why we implement a rigorous training and qualification process for every team member before they begin delivering services.

Our comprehensive training program combines theoretical and practical components to equip our staff with the skills needed to excel in their roles and align with industry standards.  In addition to initial training, we offer ongoing professional development to all staff members to ensure that our clients receive the highest quality of service.

We have a 24/7 on call coordinator for all home and community and nursing services. 

District Nursing is provided Monday - Sunday 0830-1700hours

Community Nursing

We provide short-term and long-term community nursing services for general and complex needs. We have registered nurses and enrolled nurses with a wide range of experience including accident and emergency, mental health, and primary health, who can provide patient centred specialist nursing care in your home. The range of services include: Comprehensive care planning Wound assessment, treatment, and management Medication management and oversight Catheter care and support Continence support Subcutaneous and intramuscular injections Spinal/Ventilator care Others
