Description
We provide Needs Assessment and Service Coordination (NASC) for people with disabilities. We support Aucklanders below 65 years who have physical, sensory, intellectual disabilities and Autism.
We want to get people who are eligible into the disability system, and receiving support. We’ll do as much as we can to achieve that outcome with people, their families and the services who refer to us. For example, we can help you access assistance for personal care and household management or funding to pay someone to look after a disabled loved one so the main carer can take a break. We can also explore residential care options for people who need round the clock support or options for disabled people to live independently in the community.
We also guide people to other support and funding that might also meet their needs and contribute to their overall wellbeing. Tell us your goals, needs, abilities and resources. With your lead, we will develop a support plan that enables you to live your best life.
To find out if you can get disability supports from Disability Support Services (DSS) - Ministry of Social Development, send us a referral by completing this online application.
