Community Health Service, Social
Description
We believe that healthcare starts with our communities. Patients are what make up our communities in Auckland and as such, a targeted focus on how their healthcare can be shaped is a necessary component of what we do daily.
We have a Pacific Equity team that focuses on improving health equity and wellbeing in Primary care, for Pacific and high-needs populations. Our team works with churches, community groups, families and other providers to facilitate positive change through health promotion, education initiatives and support programmes. Current initiatives include Healthy Village Action Zone (HVAZ), the Positive Parenting Active Lifestyle Programme (PPAL) and the Pacific Community Navigator Programme. The Pacific Equity team work across the Metro-Auckland region.
We have strong connections with many Pacific Providers across health and social services, and maintain a close relationship with the following Pacific General Practice Providers within our practice network:
- Langimalie Integrated Health Centres (Onehunga, Panmure & Kelston)
- Baderdrive Doctors (Māngere & Manurewa)
- South Seas Healthcare (Ōtara)
- Cavendish Doctors (Manukau)
- Southpoint Family Doctors (Manukau)
