Community Health Service
Description
Auckland PHO provides free education and support programmes for people with diabetes through Diabetes Self-Management Education (DSME) workshops.
The workshops are designed to support people with type 2 diabetes and their whānau, and are part of a DSMS programme provided by Auckland PHO and funded by the Auckland District Health Board. Our workshops are delivered free across Auckland communities on behalf of Procare, National Health Coalition and Alliance Health.
Our motivating one-day workshop aims to build people's knowledge, confidence, and skills around managing diabetes. It covers a broad range of topics, including:
- What is type 2 diabetes?
- Complications of uncontrolled diabetes
- Nutritional information on healthy eating (including portion sizes, food groups + carbohydrates)
- How to read food labels to make informed choices
- Medications
- Breaking down lab results
- Podiatry and foot care
- Physical activity
- The effect of stress on diabetes
- Goal setting
While there are objectives and topics to cover, information is tailored as much as possible to the group. The workshops are led by our trained facilitators. They are informative, conversational, and utilise adult learning techniques.
We run different language and ethnic based workshops including Chinese (Cantonese/Mandarin), Hindi, Pasifika and Kaupapa Māori.
If you would like to run a free workshop exclusively for your patients, clinic or organisation please get in contact.
Why refer to our Diabetes Self-Management support programme?
Living with diabetes can be scary and confusing and feedback from our course participants is extremely positive; patients feel they are less afraid and more empowered having gained the knowledge and skills to make some major lifestyle changes with a better understanding of diabetes.
Diabetes education workshops are:
- Evidence based
- Run by trained facilitators
- Increases people's knowledge and awareness of diabetes self-management
- Empowers people to self-manage through information and peer experiences
- Facilitates lifestyle changes & health beliefs
- Improves patient outcomes (e.g. weight, HbA1c, lipid levels, quality of life)(Deakin et al 2006; Cerriel et al 2012)
Recent feedback from an attendee to our Living Well with Diabetes Workshop:
“I didn’t choose to get diabetes but I can choose how to manage it. The diabetes workshop has supported my journey - I know that I can thrive, be happy & feel well with diabetes & still live my life.”
