Referral
Harbour Hospice accepts referrals from GPs, specialist medical practitioners, other healthcare professionals or community agencies, public and private hospitals.
Healthcare professionals can make a referral online through:
- website - https://harbourhospice.org.nz/refer-a-patient/
- Medtech or MyPractice programmes or
Family and whānau can self refer by emailing us at
