Access is usually by referral from a health professional.  Patients or family/whānau can self refer and will be assessed against eligibility criteria. 

1. Have a documented advanced disease of progressive life limiting condition and nearing end of life

2. Resident within catchment area

3. Have an extraordinary level of need (physical, psychological, spiritual) that exceeds the resources of the primary palliative care provider

4. Patient has agreed to referral if competent, or their EPOA or NOK

5.  Disease specific indicators are present
