The extended care team work closely with General Practice to support their patients in the community.

Our commitment to partnership with local iwi, social and health providers ensures that our team are able to access appropriate services for each individual and their whānau.

We are a FREE service but we do require a referral from your GP or practice nurse. Please phone your GP surgery to discuss our service as an option.

Social Worker

Our social worker works with clients in the home or at clinics as required. The service provides advocacy and inter-agency service coordination.

Clinical Pharmacist

Our clinical pharmacist provides support for medication management of high care needs patients.
