Description

Poutiri Charitable Trust was established in 1997 to contract and develop Māori Health Providers within the Bay of Plenty region to deliver on a variety of health and wellbeing services.

See below for the Community Nursing Services provided under Pouwhenua. Poutiri Charitable Trust also delivers Mental Health and Addiction in Te Puke through our Poutangata Services and a general health and wellbeing service through our Whānau Ora service.

Fees and Charges Description

This is a free community-based service to all eligible clients.  You must be a NZ Citizen or Resident to access this service. 

Hours

9:00 AM to 5:00 PM.

Mon – Fri 9:00 AM – 5:00 PM

The nursing service area covers Te Puke, Rangiuru, Manoeka, Pukehina, Pongakawa, Maketu and Otamarakau.

Please refer to Poutiri Wellness Centre listing for our medical centre hours over the Christmas and New Year period.

Public Holidays: Closed ANZAC Day (25 Apr), King's Birthday (3 Jun), Matariki (28 Jun), Labour Day (28 Oct), Auckland Anniversary (27 Jan), Waitangi Day (6 Feb), Good Friday (18 Apr), Easter Sunday (20 Apr), Easter Monday (21 Apr).

Services Provided

Community nursing

This service provides support for patients living with two or more chronic conditions for example, heart disease, cancer, gout, deep vein thrombosis, and those requiring medication education. Our nurse provides education and follow up visits before and after surgeries or treatments, and is able to support clients to attend specialist, GP and WINZ appointments. Our nurse will provide education on your illnesses in a way that you understand in a place you are comfortable including on local Marae, in your home, in our consult rooms, or anywhere else that suits you. Our nurse is able to refer you to other specilaised services such as podiatry, dietician, green prescription, district nursing, Support Nett, Occupational therapy, respite care, hospital services, mental health and addiction services and our Whanau Ora service. The Pouwhenua service can also support whanau to register with a GP service in your area and to use your community as a resource. The nursing service area covers Te Puke, Rangiuru, Manoeka, Pukehina, Pongakawa, Maketu and Otamarakau.
