• Advocacy and support (which may include transport and/or support to attend health related appointments).
• Facilitation of health promotion and self-management programmes.
• Enrolment or re-engagement with general practice, promotion and support for attending general practice screening and management programmes.

We aim to: promote whānau health and reduce health inequalities.

1. Self or whānau
2. General practitioner
3. Community providers
4. Secondary health providers
