WellSouth is the local primary health organisation (PHO) for Otago and Southland.
Our specialist Long Term Conditions Team helps your general practice look after people with long term conditions like diabetes, heart disease or chronic obstructive pulmonary disease.
The team includes Community Dietitians, Community Nurses and Clinical Pharmacists.
Long Term Conditions Nurses are involved in training general practice staff so they can deliver the best care for you. They provide advice and support to general practice staff. For example, if you work a variable shift and you are starting insulin, the Long Term Conditions Nurse can advise the practice on how best to manage your insulin alongside your shift work.
The Long Term Conditions team delivers DESMOND (Diabetes Education) and Walking Away (from Diabetes) as group courses to help improve the health outcomes of people who are at risk of or have Type 2 diabetes.
