Referral Process
If you would like to refer yourself or a young person to our programme please fill out the following Referral Form.
To be eligible for public funded service, referrals should be between 12 and 19 years old (some flexibility is acceptable), be residing within the Northland DHB region boundaries, and have bio-psycho-social health issues, mental health issues or alcohol and other drug use issues. Service entry will be managed promptly. There is no waiting list. Prioritisation considers safety and risk issues. If you're not sure, please don't hesitate to call us in confidence.
