Referral Process

Anyone aged 18 or over who has a mental health diagnosis is eligible for this service.

You can access this service through your community mental health nurse or by completing a referral form here. This service is provided in Whangārei and Kaipara.

Description

Our Community Support service is an intensive and flexible mobile support service for adults aged 18 years and over who experience mental illness. The service is a wrap around one for individuals living in the community. Support Workers are client-centred, working alongside individuals to manage their own well-being.

In addition to supporting individuals to manage their well-being, Community Support Workers can assist with the following:

  • Widen social circles
  • Find and maintain suitable housing
  • Liaise with other services that are supporting you
  • Involve whānau in your well-being journey
  • Cultural support through Arataki’s kaumātua and kuia
  • Spiritual support
