Referral Process

Most of our referrals come from the District Health Board's mental health teams. However, if you have any questions or to find out if this service is available in your area, please contact the Emerge Aotearoa office nearest you.

Description

Physical and mental health are linked in more ways than many people realise. This means that taking care of one can often make the other much easier to handle.

At Emerge Aotearoa, we work with clients to educate them about the importance of leading a healthy and active lifestyle, and equip people with the right tools for them to make positive long-term changes in their lives to help them achieve it.

We provide support to our clients that promote wellness, early detection and early intervention to those with mild to moderate mental illness and or alcohol and drug issues.

Services we provide include:

  • Assessment
  • Treatment using a range of brief, evidence based interventions
  • Use of a client-centered, collaborative model of care
  • Brokerage with other health and community services
  • Seamless transition between services if required
  • Education of service users and family/whānau
