Referral Process

To access this service for the first time, referral must come via Needs Assessment from an approved organisation. These include Te Whatu Ora - Mental Health Needs Assessment & Service Co-ordination Service,  and Synergy Wellness. The Needs Assessment will establish eligibility for entry and duration of support.

Description

Koputai Lodge provides community support to people in the West Harbour area who are living in their own homes but need additional assistance, usually a few hours per week.
