How to access Community Support

You can be referred by:

  • your General Practitioner;
  • other health professional;
  • Work and Income

Once your application has been received you will be contacted to arrange a time to meet and discuss your needs.

Support in the Community Service is discreet and confidential.

Description

WHARE HAUMARU | SUPPORTED COMMUNITY LIVING 

Whare Haumaru support people in emergency housing experiencing homelessness, who may have physical and/or mental health needs, and other barriers such as bad credit, making it difficult to access sustainable and healthy homes. Our team support you to develop a plan that is person-directed with a focus on finding a home and addressing health and social needs. 
A referral from your Work and Income Case Manager is required.

​COMMUNITY CONNECTORS 

Our Community Connector team supports people with Covid who are unable to access essential items while isolating, such as food and prescriptions.

We also support people who are not isolating but have been affected by Covid who need access to, or support from social, education, budgeting, medical, or mental health services.  We can connect them with support services and advocate on their behalf.
