Referral Process

Accepts self-referral, GP or community agencies for individuals, aged 18 years plus, in Ōtaki, Foxton, Te Horo, Levin and Shannon.

Existing consumers/tangata whaiora are encouraged to contact their key-worker in the first instance.

The service provides culturally appropriate assessments and treatment for adults from 18 years plus, suffering from a moderate to severe level of mental illness. The Horowhenua team is a multi disciplinary team that aims to provide a free quality recovery focused service. 

Supportive care/treatment during acute phases of unwellness, medication prescribing and monitoring, Individual therapy e.g. DBT (Dialect Behaviour Therapy), CBT (Cognitive Behaviour Therapy), relapse prevention planning.  recovery focused work with individual clinicians.

During business hours, MidCentral Health Community Mental Health Teams offers a mental health emergency service in their area.
