Formerly MidCentral DHB - Specialist Primary Mental Health and Addiction Service

This service is interdisciplinary, working in partnership with primary, non-government and iwi health services to deliver a programme of mental health and addiction liaison (information, education and advice) and to support navigation for whaiora with long-term mental health and addiction conditions, across Tararua, Manawatu and Horowhenua. 

Staff

The team comprises of professional, qualified, and trained staff that include Registered Nurses, Specialty Clinical Nurses, Consultant Psychiatrist and Social Workers.

Non-acute specialist Mental Health & Addiction Community based support

This service provides specialist oversight of the general family practice mental health partnership care programme (previously shared care programme). Clinical kaimahi work with our community partners to provide timely support, role modelling, specialist advise (liaison) and consultation for whaiora under partnership care. Alongside this if primary practitioners need non urgent liaison and/or consultation for whaiora outside of the partnership programme this can be undertaken without referral to locality based mental health and addiction services. This service will have a close relationship with our Te Ara Rau Programme to provide early and easy access to primary mental health and addictions services being developed in our community as well as key education and direct support for general practitioners, nurse practitioners, other health practitioners and practice nurses. Health and Wellbeing Hui recovery programmes in partnership with our primary providers will be designed/co designed by this team.
