Referral Process
Te Aka Hono Tahi accepts referrals from a wide range of sources including but not limited to GPs, schools and self-referrals.
Description
Te Aka Hono Tahi (via Ngaa Ringa Awhina) is the contracted Single Point of Entry into child and adolescent mental health services in the Hamilton Central Cluster. Te Aka Hono Tahi undertakes screening of children and young people where there are concerns about their mental health, behaviour and any other co-existing problems (CEP).
Following an assessment, where indicated, Te Aka Hono Tahi will on-refer the young person and their whānau to the most appropriate specialist service. If specialist services are not required, Te Aka Hono Tahi will suggest other services which may better meet whānau needs.
In addition, Te Aka Hono Tahi provides consult liaison support services to professionals working in the Hamilton Central region.
