Referral Process

We have an open referral process.  You can download and complete our referral form (attached below) and email to referral@poutiri.org or drop in to our offices.  Our staff will endeavour to contact you about your referral within three (3) working days. 

Description

Our community based clinical service provides comprehensive assessments and treatment for adults with co-existing disorders (Mental Health and Addiction).  We work collaboratively with our Whānau Support service to provide wrap around support for wider whānau.

We value self-determination and work alongside people to identify their goals.  Whilst it is important to deal with the past, the recovery journey is not about getting back to normal but rather finding the individuals new normal.
