Referral Process

Access is generally by referral from primary practitioners, school counsellors, Child, Youth and Family (CYF) services or professionals in other sectors.

Self-referrals or referrals made by family and whānau of the infant, child or adolescent will also be accepted.

Description

The Kaupapa:

  • To support whaiora, families/whānau and carers in maximising the individual developmental potential and mental health of infants, children, adolescents and youth between the ages of 0-19 years up to their 20th birthday
  • To establish a strong foundation for ongoing mental health and wellbeing
  • To be infant, child, adolescent and youth centered
  • To ensure family and whānau participation

Service Delivery includes:

  • Initial Assessment
  • Informed Consent
  • Recovery Plan
  • Relapse Prevention Plan
  • Discharge Planning

When an assessment identifies needs that cannot be met by the service criteria, the service will provide advice/referral to other services that are resourced to meet those needs however, where appropriate, joint care and planning between services will be used to ensure your needs are identified and responded to.

Consumers/tangata whaiora and their whānau shall be encouraged to participate in evaluation/review at each step.

Referral to CYF may be made if there are any care and protection issues.

Te Tomika Trust recognise the value of supporting children, adolescents and youth in maintaining their attendance at school and/or employment where possible and will collaborate and liaise with education and employment personnel.
