Referral Process

Clients may refer themselves to the Community Mental Health Team or be referred by GPs, relatives, friends or other community agencies. An assessment will determine whether the clients are suitable for our service. Should the client be unsuitable, he/she will be referred to an appropriate agency.

Please note: This service is for people in or residing in the Golden Bay area.

Description

Following an assessment, a case manager is appointed. Together the client and the case manager will devise an individual treatment plan. Support is offered to the client according to individual needs and may be on a short or long term basis.
