Who is this service for?

People 18 years of age and older, receiving services from Mental Health Services can be referred to the Needs Assessment and Service Coordination Service.

Description

This service assesses and coordinates resources for adults who require overall support. It is available to clients already enrolled in existing mental health and addiction services. Our team works holistically alongside the client and whānau to support recovery and rehabilitation skills, assess, and coordinate community care and support.
