Community Access Pathway (CAP) is the single point of entry for Community Support Services for Canterbury. The CAP group comprises: Comcare Trust Stepping Stone Trust Purapura Whetu Mental Health NZ (over 65yrs) Kakakura Health Emerge Aotearoa Pathways Health The Community Mental Health Support Worker is able to support the client to: Develop good household routines Work on financial skills and budgeting Improve relationships with family/friends Consider housing needs and assist in accessing the appropriate agencies for assistance Develop a healthy living lifestyle The Community Mental Health Support Worker is a skilled professional. The CAP group also manage Home Based Support and Personal Cares for the Canterbury region.

Referrals must come via specialist mental health services or your GP, using the referral form below. Please ensure all information on the form is completed and any additional information is attached.

The referrer must gain client consent before making a referral to CAP.
