The Integrated care co-ordination team (ICCT) is part of the community mental health services of Waikato District Health Board (DHB). The team supports people with long term mental illnesses to transition from specialist mental health services to primary care where their mental and physical health care needs can be met in an integrated way. For people who need ongoing support from mental health services, we offer a physical health care funding pathway that assists people to have their physical health care needs met in primary care. The team coordinates the transition process by working closely with primary and secondary mental health services, social services, NGOs, the service user and their family/whānau. Funding is available to cover the costs of accessing GP and practice nurse visits for 12 months, and during this time the ICCT provide consult liaison support and advice to the primary care team.
