Description

Mind+ provides Te Whatu Ora funded, individualised, community based support to those wanting help with their Mental Wellbeing. 

We are a person centered service working alongside individuals, their whānau, primary (GP) and secondary services (Hospital based) and anyone else an individual considers to be important in their wellbeing journey. 

Staff

Mind+ is staffed by an experienced and diverse team of mental health and addictions certified "Wellness Advocates".  They include those with lived experience, professional skills and training, along with registered Allied Health and Nursing Clinicians.

Individualised Community Support

Mind+ provides Te Whatu Ora funded, individualised, community based support. We work alongside individuals, their whānau, Community Mental Health and Addiction team and anyone else an individual considers to be important in their journey. The aim of this work is to empower individuals to be in charge of their own health and wellbeing with support to identify and plan for their dreams and aspirations. The level of support varies according to individual needs over time e.g. from 1hr per month to 2hrs per day. This support is provided to people living anywhere in the wider Te Puke area.

Referral Process

Referrals are accepted from:

  • Individuals, whānau, social or other social/communuity services who identify a (non-acute) need around a person's mental wellbeing. These funnel through our regional "Navigate" pathway which involves a number of local NGO providers.  They can also come through our website.
  • BOP Community Mental Health and Addiction Needs Assessment Service Coordinators (NASC)
  • Te Puna Hauora Needs Assessment Service Coordinators
