Referral Process
Ngāti Kahu Social and Health Services provide support to primary and secondary health services and so essentially referrals that are made to us are from Child Youth Mental Health and or Adult Mental Health Services.
Our role is to work with who make referrals to ensure we develop a plan with our whaiora so that they are supported in the community. In addition, we are able to receive self referrals where we will complete the relevant assessment to assess what services need to be engaged.
Description
Kai mahi from within our roopu provide support to tangata whaiora and whānau whose lives are affected by mental health and/or addictions.
Our Whaiora kai mahi help with:
- completing an assessment for services
- informing, enabling and empowering whānau
- accessing information to assist their whānau
- entering employment or training
- gaining/regaining relationships with their whānau
- developing new skills and capabilities
- provide advocacy where needed
