Programmes
This service provides specialist oversight of the general family practice mental health partnership care programme (previously shared care programme). Clinical kaimahi work with our community partners to provide timely support, role modelling, specialist advice (liaison) and consultation for whaiora under partnership care. Alongside this if primary practitioners need non urgent liaison and/or consultation for whaiora outside of the partnership programme this can be undertaken without referral to locality based mental health and addiction services. This service will have a close relationship with our Te Ara Rau Programme to provide early and easy access to primary mental health and addictions services being developed in our community as well as key education and direct support for general practitioners, nurse practitioners, other health practitioners and practice nurses. Health and Wellbeing Hui recovery programmes in partnership with our primary providers will be designed/co designed by this team.
