Description

The Tararua Locality Community Mental Health and Addictions Team provides services covering Dannevirke, Woodville, Pahiatua and outlying rural areas. They work with Whaiora (people needing services) and their Whaanau (family), general practitioners/nurse practitioners and other treatment providers to help provide care and promote recovery for adults needing specialist mental health and addiction treatment in our community. 

What we do 

The Tararua Community Mental Health & Addictions team provide: 

  • Responsive advice, assessment, treatment and therapeuic interventions for people who require specialist mental health and addiction care.
  • Support to other service providers through collaboration and education.
  • Assists with connection to support and advocacy services for people with mental health & addictions needs. 
  • Access to specialist cultural and peer expertise and support.

Tararua Mental Health and Addiction Locality Service

The service provides culturally appropriate assessments and treatment for whaiora experiencing a moderate to severe level of mental illness. The Tararua team is an interdisciplinary team consisting of Whaanau Ora Kaitautoko (support staff), Peer Support Specialists, Nurses, Social Workers, Psychiatrist, Addiction Practitioners and has access to Psychology. Special Authority Renewals for methylphenidate and dexamphetamine are no longer processed through Specialist Mental Health and Addiction Service. Requests for assessment to diagnose adult ADHD is not within the remit of Specialist Mental Health and Addiction Service. Parameters for assessment within the service relates to people who experience moderate to severe functional mental health and addiction risks and needs. The following resources/information may be a helpful to attain an adult ADHD diagnosis: ADHD medicines – adults | Healthify Further advice and guidance relating to ADHD might be accessed by liaison with the ADHD Association (www.adhd.org.nz).  ·Individuals retain the option of independently referring themselves to a psychiatrist, on a private basis.  Access for a private consultation for a diagnosis and treatment options can be sourced at cost to the patient via www.anteris.nz , Home | The New Zealand TelepsychNetwork or another private provider.

Referral Process

Accepts self-referrals, GP or community agencies for individuals, aged 18 years plus, in Woodville, Pahiatua and outlying rural areas. Our team work to support navigation of children, adolescents, adult and older adult whaiora whānau to appropriate services.

Existing whaiora whānau are encouraged to contact their care coordinator in the first instance.

The programme provides a responsive urgent mental health & addictions triage and assessment, and interventions for people who are acutely mentally unwell. People accessing this service will be assisted to navigate their plan of care to the services that will best meet their needs and as close to home as possible. This locality-based programme is provided by experienced mental health and addiction practitioners who work within the centre. This is an 0800-1630 service only.

Description

The programme provides a responsive urgent mental health & addictions triage and assessment, and interventions for people who are acutely mentally unwell. People accessing this service will be assisted to navigate their plan of care to the services that will best meet their needs and as close to home as possible. This locality-based programme is provided by experienced mental health and addiction practitioners who work within the centre. This is an 0800-1630 service only.

Flexible Assertive Community Treatment (FACT)

This programme provides longer term treatment for people with complex mental health & addiction conditions; and provides care within a Flexible Assertive Community Treatment model (FACT). FACT aims to ensure continuity of care, to prevent admissions to hospitals and to foster inclusion, so that whaiora can maximise independence and be connected to community and whānau supports.

FACT aims to ensure continuity of care, to prevent admissions to hospitals and to foster inclusion, so that whaiora can maximise independence and be connected to community and whānau supports.
