The Te Manawanui service centres on two new roles:

Kaiw'akaako-a-'auora
Health Improvement Practitioner (HIP)

Kaitautoko
Health Coach/Cultural Support Worker

A Kaiw'akaako-a-'auora / Health Improvement Practitioner (HIP) is a registered health professional who works alongside GP’s, nurses and Kaitautoko helping out when any problems with living and stress are having an impact on our hauora (health and well-being). Kaitautoko / Health Coach & Cultural Support Worker partner with people experiencing issues that impact on their health and wellbeing to support them to identify their own priorities, set goals for change and to develop a plan to address those goals through developing self-management skills and linking them to resources and supports. 

For example, a HIP can practically help with:

  • Stress/nervousness/worry
  • Low mood/anxiety
  • Chronic pain/long-term conditions
  • Diabetes/pre-diabetes
  • Fatigue/tiredness + sleep problems
  • Reducing/quitting smoking
  • Weight loss
  • Feeling stuck/unmotivated
  • Relationship issues
  • Children/youth behavioural issues
  • Any other health/life problem

The five key roles of the Kaitautoko are:

  • Self-management support
  • Acting as a bridge between clinician and person/whānau
  • Navigation of the health and social services system – including linking to appropriate community supports
  • Emotional support
  • Providing continuity within a busy general practice team

Te Manawanui - Integrated Primary Mental Health Initiative

We have launched a new Integrated Primary Mental Health & Addictions service, Te Manawanui, that is embedded within primary healthcare (general practice). The Te Manawanui service is designed around Taranaki Māori communities but it is for everyone. The service aims to provide support that is culturally responsive, accessible, practical and convenient so that it uplifts hauora (health and well-being) of individuals and their w'ānau. Te Manawanui addresses the thoughts, feelings and behaviours that underlie poor health, integrating mental health and physical health needs. Individuals may benefit from Te Manawanui to support issues including anxiety, whānau relationships, stress, depression and lifestyle choices. Te Manawanui is a whole of health approach, enabling increased access to people at the time and level that it is needed.

There is no eligibility criteria to access the service which is delivered in selected General Practice (GP) clinics and also in the community.
