My Vaccination Documents - get in person

Our service can help you request and print the following documents:

• International Travel Vaccination Certificate
• Your vaccination record

• Bring your Photo ID and Covid-19 Vaccination Appointment Card (Purple Card) into pharmacy.
• Fill out the Vaccine Pass Request form and Pharmacy staff will verify your identity and details are correct.
• Physical copy on paper can be printed for free.
• If you want yours to be laminated into a card, there is an extra $3 service fee for lamination.
• Electronic copy can be sent to your email address
• Please allow 24-48 hours to staff to complete your request.
