You need to bring to your appointment:
- Referral letter including radiology report/s
- Patients being seen for hip pain, please request printed 'full size xray films' and bring to appointment
- A list of your medications
- Your ACC number and Date of Injury if appropriate
- Your Health Insurance Membership Number if appropriate
Depending on complexity, the initial fee will range between $220.00 and $290.00 including GST.
