Individual education

Please ask your healthcare professional to refer you to the CMH Outpatient Dietitian clinics if you are overweight and also have one or more of the following conditions:

• Cardiac (risk > 15%, or existing CVD or heart failure)
• Requiring weight loss prior to surgery (not bariatric surgery)
• Age <25 years
• Severe learning disability
• Requires English language interpreter

If you require support for weight loss, healthy eating advice and/or physical activity and DO NOT have one of the conditions list above, please ask your healthcare professional to refer you to Green Prescription for support.
