How do I access this service?

Referral

We accept individual referral from services or clinicians that meet the following criteria:

  • Missed clinic appointments at least twice or more over the past year
  • Complex medical background and/or multiple service involvement
  • Unable to navigate treatment plan
  • Missed bookings
  • Clinical need with language support and requires ongoing education support
  • Cultural need with a clinical component
  • For cultural need only, refer to the Tautai Fakataha team https://www.healthpoint.co.nz/public/other/auckland-dhb-pacific-health/
