Description
If you or a loved one needs nursing care or other care services in the community, you may be referred to the Community Care Coordination Service to connect you with the most suitable service.
What is Community Care Coordination?
The Community Care Coordination Service aims to provide a single point of access to navigate and co-ordinate services for people requiring short term health and enablement services. We will link you with the most suitable service without you needing to be assessed by multiple services.
What to expect:
If you need Community Care Coordination, you might be referred to our service by a medical professional who is supporting you e.g. a nurse, social worker, occupational therapist or your GP.
When our team receives a referral, we will work to connect you with the right service to support a range of needs, see our services below:
If you have left hospital and need further support, we can help put short-term services in place for 4 – 6 weeks after hospital discharge. We will follow up with you after four weeks to check how you are doing.
GPs can also refer you to our service without you having been in hospital, you have had a serious health issue/event. We will support you into appropriate services, to help you avoid needing hospital treatment.
How do I access this service?

Referral Expectations
Referrals to the Community Care Coordination Service come from registered health professionals including nurses, social workers, occupational therapists, physiotherapists and GPs.
