Our Needs Assessment Service staff assess and coordinate supports for people who struggle with daily activities because of a health condition or disability.

The needs assessment service comprises two teams:

  • Health of Older People - Over 65 years (HOP)
  • Disability (Support Works)

Health of Older People - We develop a meaningful plan to help people stay safe and independently living in their home. We can refer to other services and agencies and link people with community support providers. We also complete assessments that inform the decision to move into Aged Residential Care. Another funding stream is supporting children and younger adults living with a Long Term Chronic Health Condition.

Disability - Support services are available to people with a physical, intellectual or sensory disability, including Autism Spectrum Disorder under the age of 65 years which is likely to continue to six months and requires ongoing support to live independently. We work alongside the disabled person to develop a plan that supports them to build a good life in a community that values who they are and what they have to offer. We can refer to other services and agencies to support the disabled person to achieve their plan.

Referrals can be sent by GPs or another health care service or provider. People may self-refer or be referred by family or someone they know with their consent.

Referral Expectations

Health of Older Persons - We will have a conversation with the person when a referral is received and arrange to complete an assessment with the person. We will liaise with other health clinicians in the person's circle of care to pull together a collaborative and shared approach to goals and outcomes.

Disability - The National Referral Centre will review referral information for eligibility to disability support services. Once accepted it will be referred to Support Works and our team will contact the disabled person/family to arrange a time to meet and develop a My Plan. As part of the plan we may discuss what funded services and supports are available within our region. If a referral is not accepted as eligible the National referral centre will advise the person who has completed the referral and recommend any alternative pathways of support that may be more appropriate.
