Description

Home based support services can be Short-term or Long-term.
The purpose of the Short-Term Home and Community Support Services (ST-HCSS) is to promote and maintain independence of people who are experiencing difficulty caring for themselves because of an illness, acute medical condition, a result of post-acute hospitalisation or an injury. This service enables clients to remain in their own home or other private accommodation in the community or return to their home as soon as practical, by providing services that support and sustain activities necessary for daily living in a way which promotes the client’s independence and quality of life.

ST-HCSS are provided on a short-term basis only – generally 2-6 weeks (8 weeks maximum).

People who need supports beyond 8 weeks are reviewed and assessed for Long term support needs. The service ensures rapid access to home care support and more accurate assessment in the community so the patient's needs are met in the right place, at the right time.

We provide:

  • Short term assistance with showering, basic housework and other tasks identified by your assessor
  • Equipment to improve your safety
  • Assessment for Meals on Wheels

What to expect?

  • To best understand your needs, an assessor will visit you at the hospital or home and complete an assessment
  • Your assessor will let you know what assistance can be provided
  • The Carers allocated to you are employed by Auckland District Health Board – NASC will review this regularly
  • Your needs will be reviewed, and changes made as necessary
