Description

All hospital inpatients need medicines as part of their treatment plan - whether for acute illness, chronic disease management, symptom relief, prevention of complications, or perioperative care - meaning timely access to safe, appropriate and optimised pharmacotherapy is essential to high-quality hospital care.

The Inpatient Pharmacy Service exists to ensure safe, effective and efficient use of medicines across all inpatient settings. The service supports improved clinical outcomes, reduces medication-related harm, enhances patient experience, and enables hospital flow through accurate medicines reconciliation, evidenced-based and patient-focused clinical review and timely discharge support.

Inpatient Dispensary:
Middlemore Hospital Pharmacy supplies medications to all inpatients of Middlemore Hospital and other Counties Manukau Health facilities (Manukau Health Park, Pukekohe Hospital, Auckland Spinal Rehabilitation Unit).

Clinical Pharmacy - Clinical pharmacists provide a clinical pharmacy service to inpatient areas at Counties Manukau. They work collaboratively with patients/whaanau, doctors, nurses and other health professionals to ensure safe and quality use of medicines.  Clinical pharmacists establish medication histories, review medication charts, optimise and monitor drug therapy, provide patient/whaanau medication counselling, as well as facilitate supply and access to medicines.

Medicines Information Service - The Medicines Information Service consists of a team of experienced pharmacists who are available to answer medication-related enquiries for health professionals in the Counties Manukau district. The team are available on Monday to Friday from 8am - 4:30pm.

Community/Outpatient Pharmacy: Haumanu Pharmacy is located within Middlemore Hospital in Pataka Place.

Referral Expectations

Clinical Pharmacists visit inpatient areas and some outpatient clinics to provide medicines management services and advise on pharmaceutical care.  Patient services such as medication review and medication counselling are part of this process.

Patients should bring any current medication (tablets, creams and/or inhalers) into hospital with them on admission as inpatients.  Medication lists, such as yellow cards, are also encouraged.  This ensures that the optimal information about medication being taken is available to the health team at the time of admission.  Pharmacists may call a patient's GP or regular community pharmacist to confirm or clarify differences in medication history.
