Description
Our Pacific Health Service
We look forward to working together with our Pacific patients and their families, achieving better health outcomes and equity for our Pacific people by:
- Promoting the importance of addressing health conditions sooner to facilitate success of longer term life goals, spiritual health, and prosperity of family and community.
- Empowering and supporting Pacific families to make informed decisions over their health.
We have three teams
Fanau Ola Team
- We are a team of Pacific senior nurses working in the medical specialties supporting Pacific patients in their healthcare journey.
- We work closely with wards and departments to support our medical staff to ensure we provide the best care to our Pacific patients in a collaborative manner.
- We visit specific wards on a day to day basis but can also receive referrals from services that need our support.
Cultural Support Team
- Our cultural team work closely with all wards and departments in adult and children’s services at Te Toka Tumai Auckland to support our Pacific patients and families during their journey through our hospital.
- We provide advice and guidance on the differences between Pacific cultures, their protocols and how best to support our Pacific patients.
- We visit the wards daily to see our patients and talk to the appropriate staff.
- If appropriate, we work alongside our nursing team to provide wrap around care and support for our Pacific patients and families.
Pacific Planned Care Team
- We are a team of Pacific senior nurses that are competent in our own culture and speak our ethnic language.
- Our focus is supporting Pacific patients going through the surgical pathway, from first consultation to surgery.
- We work in partnership with Pacific patients, delivering education and guidance with navigating their journey in the healthcare system, in a culturally appropriate way and often interact with our patients in their own native language
- We also work collaboratively with wards and departments within the hospital to support Pacific patients as much as possible to ensure a successful outcome and a positive experience.
