Description

The Adult General Rehabilitation Service provides assessment, treatment and rehabilitation for patients living in the Counties Manukau Health region.

Care is provided by an interdisciplinary team, that develops a patient-focused, goal-based treatment programme, based on the individual patient's needs.

The service includes inpatient care on Ward 23, Middlemore Hospital, an outpatient medical and physiotherapy service based at the AT&R Unit of Middlemore Hospital and a community stroke rehabilitation service.

Referral Expectations

Criteria for Admission for an Inpatient Period of Rehabilitation are:

  • Reside within Counties Manukau Health catchment area
  • 15 years and older
  • meet the NZ residency/ eligibility criteria
  • Medically able to participate in up to 3 hours of rehabilitation treatment per day, 5-6 day per week.
  • Willing to participate in an individualised rehabilitation programme up to 3 hours per day, 5-6 days/week. 
  • Potential for improvement and good rehabilitation outcomes
  • Requires 24 hour medical availability, rehabilitation nursing and Interdisciplinary assessment and treatment.
  • Acute investigations and acute treatments are complete. 
  • Inpatient Adult General Rehabilitation Admission diagnoses:
    All ages over 15:
  • Stroke with good rehabilitation potential
  • Major amputations- all age over 15 with good rehabilitation potential
  • Rehabilitation Diagnoses under 65 years:
  • Non-traumatic Acquired Brain Injury (those not appropriate for ABI Rehabilitation facility in Rānui);
  • Orthopaedic conditions and trauma including total joint replacement
  • Other neuro conditions including GBS and MS and non-malignant neurosurgical patients.  

Cancer-related Neuro or Musculoskeletal Rehabilitation Diagnosis Criteria for Admission:

  • Cancer treatment complete
  • Prognosis greater than 6 months, per oncologist/radiation oncologist
  • Patient and family aware of diagnosis and prognosis
  • Medically able to participate in rehab
  • Will have appropriate discharge destination and supports
  • Chooses to be admitted to rehab with understanding that a safe and supported discharge, rather than maximal return of function, is the goal.
  • Cancer-related rehabilitation may have interruptions due to medical concerns and occasionally for investigations and brief treatment, thus the “3 hour per day, 5 days per week rule” does not strictly apply.

Other Considerations:

  • Burns rehabilitation takes place at TNBC
  • Consideration for inpatient rehabilitation for patients on haemodialysis is on a case by case basis.
  • Persons with severe dementia are not appropriate for inpatient rehabilitation.
  • For those patients who will not be able to discharge to their prior abode, or who were living in a private hospital setting, inpatient rehabilitation may be appropriate to decrease their level of care after discharge. Appropriateness for inpatient rehabilitation will be assessed on a case-by-case basis.
  • Appropriateness for inpatient Adult General Rehabilitation is determined by the Rehabilitation Medicine Specialist after consultation/review.
  • Patients may be admitted from the community for assessment only such as spasticity intervention trials and MDT trials at clinician's discretion.
  • Over 65 patients with neurological conditions other than stroke will only be considered on a case by case basis. During periods of high demand, patients who require stroke and under 65 rehab may receive higher priority.
  • Over 65 patients (including stroke and amputations) from Pukekohe and Franklin should preferably consider going to Pukekohe Hospital, except when medical needs (NG tube, imaging requirements and medical interventions) dictate admission to ward 23.  Under 65 patients from Pukekohe and Franklin generally come to ward 23, though on a case by case basis there may be agreement for rehabilitation to continue at Pukekohe Hospital. 
  • Māori and Pasifika patients between 55 and 65 years may be considered as "close in age and interest" and maybe directed at times to Health of Older People (HOP) depending on the need and availability.
  • Similarly elderly patients with stroke and amputation diagnosis maybe directed towards HOP if their rehabilitation potential is limited and discharge planning and medical co-morbidity management are the focus of hospital stay.
  • At times of need, Adult Rehab service may accept spinal injury patients on a case by case basis to support ASRU. At times, it may become necessary to direct some other (eg over 65 stroke) patients towards HOP service.
  • If ACC, private insurance or funding other than MOH, must have funding approval before admission.
  • Patients from Cook Islands will be accepted for inpatient rehabilitation if their next of kin resides in the Counties district using the same criteria.
  • FND (Functional Neurological Deficit) patients will be considered on a case by case basis.
  • Those patients who are deemed to be having no further rehab potential from previous admission may be declined, despite new admission.
  • For those destined for placement with limited goals, rehab clinician may consider on an individual basis.

Fees and Charges Description

No charge for the service for New Zealand Residents.  Non residents are charged a per bed day rate.

Procedures / Treatments

Acute consultation service

Patients referred for admission to the inpatient Adult General Rehabilitation service after an acute episode are assessed by a rehabilitation consultant or rehabilitation registrar to determine the appropriateness of their admission to the service. Patients may be referred from other services in Counties Manukau Health, including the community services as well as from other DHBs for CMH eligible patients.

The broad category of patients we care for are: stroke patients patients with other neurological involvement (for example: multiple sclerosis, Guillain Barre, hypoxic/ anoxic brain injury, cancer related neurologic deficits) patients with amputations or other musculoskeletal problems such as fractures, multitrauma, complicated joint replacements patients debilitated by prolonged and/or complicated hospital stays
