About the Service
As part of Older Adults Service, the Assessment, Treatment & Rehabilitation (AT&R) Service provides a specialist inpatient and outpatient service dedicated to improving and maintaining the health and independence of older people. The service is a mix of geriatric medicine and rehabilitation for patients who often have multiple or complex needs, including those recovering from an injury or illness or who have an age related condition such as dementia or Parkinson disease.
The AT&R team help older adults manage the challenges associated with disability or aging that affect their day-to-day living. Working with the patient and their family/whānau, we plan their care and help the patient achieve their healthcare goals. The team may include doctors, nurses, physiotherapists, occupational therapists, speech language therapists, social workers, dietitians, pharmacists and needs assessors, depending on the patient's health needs and goals.
There are two inpatient wards at North Shore Hospital (Wards 14 and 15) and one at Waitakere Hospital (Muriwai Ward). We also provide an outpatient clinic and a domiciliary visiting service to patient’s homes and aged residential care facilities.
Referral Expectations
AT&R receives referrals from the primary care sector and the community via the Gerontology Nursing Service. In addition, the service receives referrals from acute hospital wards and the Emergency Department.
Entry Criteria
AT&R accepts patients who:
- Are over 65 years of age (Māori and Pasifika 55 years and over). Note: People 55-65 years, with age related conditions, are considered on a case-by-case basis.
- Require a period of rehabilitation, particularly frail older adults, who are at risk of losing their independence or are having difficulty managing at home.
- Need multidisciplinary specialist assessment and treatment for conditions or disabilities related to age and frailty (for example – mobility, continence, falls, fractures, declining cognitive function, delirium and strokes).
- Need specialist assessment and treatment for multiple and/or chronic health conditions or disabilities.
- Require ongoing assessment for safety and community support at home including follow-up by home or community-based services.
