All referrals for a Geriatrician assessment or the POPs service must come from your General Practitioner (GP).
Anyone can ask for a Needs Assessment to look at how they can be more able to manage in the community. However, if they have never been seen by the Health of Older People service or if there has been a significant change in their capabilities, a new medical review by a GP, Practice Nurse or specialist will be required.
