The Aged Care Needs Assessment and Service Coordination is a district-wide service operating across Dunedin, Invercargill, and the rural hospitals.
The service undertakes comprehensive assessments to determine an elderly person’s level of need for ongoing support within their home and community. Based on this assessment, appropriate services are allocated to meet those identified needs.
Support is provided to individuals who have functional limitations related to ageing, or those with long-term chronic health conditions with significant or complex needs. People under the age of 65 may also be eligible if their disability is related to an ongoing health condition.
The service is available Monday to Friday, from 8:30 am to 4:30 pm.
