About the Service

The Early Discharge and Rehabilitation Service (EDARS) is a service at Te Whatu Ora Health New Zealand - Waitematā that delivers individually prescribed rehabilitation based on a patient’s specific needs and delivered in their home environment. It comes under the umbrella of the Allied Health Service and is an alternative to admission to an inpatient Assessment, Treatment & Rehabilitation ward (AT&R), or can shorten a patient’s stay in hospital.

EDARS care is comparable to the current AT&R inpatient service and is delivered in the patient’s home by a team of senior clinicians across the disciplines of: social work, dietetics, speech-language therapy, occupational therapy, physiotherapy and nursing, and in collaboration with our geriatricians.

 

Entry Criteria

To be eligible for admission to EDARS patients must fulfil all of the following requirements:

  • Be resident within Te Whatu Ora - Waitematā catchment area
  • Currently be an inpatient of Te Whatu Ora - Waitematā, this includes regional services such as the Auckland Spinal Rehabilitation Unit
  • Be aged 55+ years Māori or Pacific, or 65+ years all other ethnicities
  • Have a medical condition that is sufficiently stable to be safely discharged home
  • Have the potential to return to safe function following short-term intensive rehabilitation
  • Be physically and cognitively able to engage in a short-term intensive rehabilitation programme
  • Have a home environment that is conducive to supporting a short-term intensive rehabilitation programme.

7:30 AM to 4:30 PM.

Mon – Fri 7:30 AM – 4:30 PM
