Lakes DHB - Pokapū o te Taiwhenua Network

The Pokapū o te Taiwhenua Network is an inclusive virtual network of health and wellbeing community providers, community members, primary care, and specialist care supporting digital health equity and digital inclusion strategies to support strategic imperatives of health equity, whānau wellness, and integration of health and wellbeing services. 

The vision of the Network is supporting “right care, right place, right time, right technology, right facilitation.” 

COVID-19 clearly laid bare the need to close the digital divide for digitally isolated whānau and communities both for basic household needs but also to support access into virtual health offerings enabling whānau to manage their health from home.

The Pokapū o te Taiwhenua Network is in its initial stages of piloting video telehealth facilitation using a non-clinical workforce in rural localities.

As new virtual offerings become available, we will continue to update this website with links and information. 


Here are some of the digital offerings which are available now:

Video telehealth

  • Connect for your health appointment through video conference
  • Ask your health professional about the possibility of using video telehealth in future appointments when it is appropriate
  • Lakes DHB hospital services use Zoom with passcode issued from a Lakes DHB Zoom authorised license

Patient portals

  • Some GP practices offer patient portals which enable clients to manage their health.  Enquire with your GP practice team.
  • Functionality differs by patient portal but in general may allow patients to:
    • request repeat prescriptions
    • book appointments
    • view their:
      • lab results
      • GP's clinical notes
      • current diagnosis
      • medical conditions
      • medications list
      • immunisation and vaccination history
    • receive reminders and recalls from their GP practice team
    • send and receive secure messages to and from the GP Team

COVID-19 Vaccination


Rapid Rēhita

  • Allows patients to enrol with a GP practice online


Health Navigator

  • This site provides one place for New Zealanders to find reliable and trustworthy health information and self-care resources. You can find links to health conditions, medicines, and healthy living advice, apps and videos, resources in different languages, where to find support, information for clinicians and more


Find Your Midwife

  • Find midwives in the Lakes region


Smart Watches and other wearable technology

  • Many options available including monitoring steps, heart rate, stress levels and much more


E-mental health

  • Clearhead - e-mental health app for those wanting to explore their mental wellbeing
  • Helplines 1737 & Lakes Crisis Line - helpline service for anyone who needs support
  • Just a thought - e-mental health app for those with depression or anxiety
  • The Lowdown - e-mental health for rangatahi
  • Living Sober - e-mental health app for those with harmful drinking or alcohol misuse



Kanorau Digital

  • Digital coaching programme through Te Wānanga o Aotearoa

Ministry of Social Development (MSD)


Te Puni Kōkiri Marae Digital Connectivity

  • This programme is supporting marae digital connectivity


Translation Services

  • If you are needing translation services for your hospital appointment, Lakes DHB can now support translation services by video conference instead of just telephone. This service is provided through Te Aka Matua Service (Te Reo) and Decypher.


Subsidised Broadband

  • Skinny Jump provides subsidised broadband for Kiwis who don't have a broadband connection at home because cost is a barrier.


Zero-Rated Data Websites

  • The Ministry of Health's 'Sponsored Data Initiative' allows you to access key health sector websites via your mobile phone without incurring data use charges.

Broadband Map NZ

  • Broadband Map NZ shows what internet access is available by address. This can be useful in planning where video telehealth appointments will be successful from (see mobile filter). 



  • This is a modified tablet that supports clients, who may be overwhelmed with the complexity of standard issue digital devices, stay connected. 



  • This is a FREE text message programme providing information and support to pregnant women and mothers of babies and their whānau.
